The University Medical Center of El Paso Foundation (UMCF) has received a significant boost in its efforts to fight diabetes in the region. Thanks to a $422,692 grant from the Paso del Norte Health Foundation, UMCF is launching Diabetes Care Plus, a three-year care management program designed to support patients who struggle to manage Type 2 diabetes.
UMC of El Paso currently provides care to more than 11,000 patients diagnosed with Type 2 diabetes mellitus. With the help of this new funding, the hospital will be able to expand support for these individuals by integrating a clinical pharmacist into the care team. The pharmacist will collaborate directly with physicians to coordinate treatment, deliver personalized education, and ensure consistent follow-up, all aimed at improving patients’ adherence to their care plans.
“Diabetes Care Plus will enable UMC of El Paso to implement evidence-based care management strategies for individuals facing challenges with their diabetes treatment plan,” said Dr. Juan Castillo, the attending endocrinologist for the grant program.
The program will be based at UMC’s Northeast Outpatient Clinic, serving as a pilot model for delivering comprehensive diabetes care in the community. By offering personalized and consistent follow-up, UMC aims to significantly improve patient outcomes and demonstrate the impact of coordinated, patient-centered care.
With diabetes being one of the most prevalent and difficult chronic conditions in the region, Diabetes Care Plus marks an important step forward. Through this innovative program, UMC of El Paso continues to prioritize health equity and accessible care, bringing tailored diabetes management to those who need it most.
